Our Mission
To assist our clients and their loved ones with means to remain in their homes and communities avoiding placement in an institution.
Overview
WHAT IS IT THAT WE DO:
Comfort Facilitators assists families and/or individuals on Medicaid who qualify for Consumer Directive Services, needing in-home healthcare services and know someone (known as the attendant) who they like and/or trust to care for them and/or their family member(s). Comfort Facilitators will assist you in hiring that person (including yourself and/or a family member). More than one attendant can be hired for a case. Medicaid pays for these services through Public Partnership, LLC (PPL), and Comfort Facilitators will assist you with filing the necessary paperwork for the attendant, the person needing care and qualifying for one of the four approved Waivers (with Consumer Directive Services - CD) under the CD tab to your left:
MEDICAID WAIVERS WERE DEVELOPED TO:
Encourage people with disabilities and the elderly to access services in their homes and communities. Medicaid is a joint federal-state program. Medicaid Waivers provide funding to serve people who are eligible for long-term care in institutions such as hospitals, nursing facilities, and intermediate-care facilities. Through Medicaid Waivers, states can “waive” certain requirements including the requirement that individuals live in institutions in order to receive Medicaid funding.
WHAT IS CONSUMER DIRECTIVE SERVICES:
Consumer Directive Services provides programs specifically intended to allow individuals to remain in their homes and communities avoiding placement in an institution. (See Consumer Directive Page for more information.)
